D-Lactate, present in much lower quantities than the more common L-lactate is produced endogenously through the methylglyoxal pathway. This pathway generates nanomolar amounts of D-lactate, primarily in tissues with high glucose utilization rates, such as the brain and eye lens. The production of D-lactate may serve a protective function by scavenging free radicals and mitigating oxidative stress. Also produced by gut bacteria, elevated D-lactate levels may indicate intestinal barrier dysfunction, dysbiosis, or increased intestinal permeability. This could have implications for diagnosing and monitoring diseases such as inflammatory bowel disease and short bowel syndrome.
