Dipeptidyl peptidase-4 (DPP4), also known as CD26, is a multifunctional protein, involved in immune regulation, signal transduction, apoptosis and glucose metabolism and has become a significant target in the treatment of type 2 diabetes mellitus. This enzyme exists in both membrane- bound and soluble forms. DPP4's function as a serine exopeptidase cleaves X-proline or X-alanine dipeptides from the N-terminus of polypeptides. This is particularly important in the regulation of incretin hormones, such as glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP). By rapidly degrading these hormones, DPP4 plays a key role in glucose homeostasis. In the context of type 2 diabetes treatment, DPP4 inhibitors (gliptins) have been developed to prevent the inactivation of GLP-1 and GIP, causing increased insulin secretion, reduced glucagon secretion, and ultimately, lower blood glucose levels.
