Fructose, a monosaccharide found naturally in fruits and increasingly used in processed foods, plays a significant role in human metabolism with far-reaching implications for health and disease. Unlike glucose, fructose is primarily metabolized in the liver and small intestine. It is absorbed in the small intestine via GLUT5 transporters and rapidly cleared from the bloodstream, with the liver extracting up to 70% of an oral fructose load. In the liver, fructose undergoes a process called fructolysis, which is catalyzed by specific enzymes including fructokinase, aldolase B, and triokinase. Fructose metabolism bypasses the rate-limiting step of glycolysis, allowing for rapid conversion to triose phosphates which has several consequences: de novo lipogenesis, increased triglyceride synthesis and VLDL secretion; conversion to glucose in the liver and increased lactate production. Fructose intake is associated with metabolic syndrome, cardiovascular disease and type 2 diabetes.
