L-malate, an intermediate in the tricarboxylic acid cycle, plays a role in cellular energy metabolism and various physiological processes. This four-carbon dicarboxylic acid is involved in several important metabolic pathways and functions: It serves as a critical intermediate in the transport of reducing equivalents from the cytosol to the mitochondria through the malate-aspartate shuttle. In the heart, this shuttle is particularly important; under conditions of limited glucose availability, L-malate can be converted to oxaloacetate and then to phosphoenolpyruvate, feeding into gluconeogenesis; L-malate acts as an intracellular anti-inflammatory metabolite, inhibiting the production of pro-inflammatory cytokines such as IL-1β, TNF-α, and IL-6 in activated macrophages. This effect is partially mediated through its ability to modulate intracellular pH; L-malate supplementation has been shown to improve physical stamina and reduce muscle damage during exercise.
